If you declined STD or LTD coverage during your initial eligibility period, you will need to complete an Evidence of Insurability Form (EOI) along with the enrollment form for the coverage you are requesting. These are confidential and mailed directly to the insurance carrier (address on form) for a determination. Your coverage and deduction will begin upon receipt of approval from Guardian Life Insurance.
Advantage Technical Resourcing has a paperless claims system with Guardian Life Insurance. At the time of eligibility, a TeleGuard card and letter are mailed to eligible employees for future reference and use. If you do not receive your information, please contact your HR Representative or Benefits Administrator.
