Guardian DHMO:
The Guardian DHMO Dental Plan is designed to offer general benefit options. In order to receive the full payment options, you must choose a Dentist "in-network." You may refer to the Guardian Life web site to review the participating Dentists included in the Guardian DHMO program or from the DHMO directory below. You must select a Primary Dentist at the time you enroll.
